Correlation between bile duct obstruction and ductal cancer found

When bile duct cancer cells were placed in the liver of animals with bile duct obstruction, they grew more rapidly than identical cells placed in animals without bile duct obstruction. In fact, half of the total liver mass of the rats with bile duct obstruction became replaced by cancer cells within three weeks compared to only 16 percent of that of animals without bile duct obstruction.

   Oregon Health & Science University researchers are reporting the discovery of an early survival advantage when a combination of surgery and radiation therapy is used for patients with a rare but deadly bile duct cancer. "Surgery and radiotherapy shows an early response benefit. It shows that the addition of radiation is potentially reasonable for the first-line treatment," said Clifton David Fuller, M.D., Ph.D., principal investigator of the study.

   Although approved by the US Food and Drug Administration as a palliative treatment for cancer patients who have developed bile-duct obstructions, biliary stents are sometimes used "off-label" for the treatment of peripheral vascular disease. A study in today's issue of the American Journal of Therapeutics finds that off-label use of biliary stents is increasing, and that the majority of adverse events and device malfunctions associated with the use of these stents occurs during off-label usage.

   Endosonography-guided biliary drainage is a new method that enables internal drainage of bile. A research group led by Professor Naotaka Fujita in Japan performed histological examination of and around the fistula created by ESBD in a patient with ampullary carcinoma. No hematoma, bile leakage, or abscess was observed at the site of puncture in the duodenum and bile duct wall and in/around the sinus tract. These findings may encourage more active use of ESBD in cases with failed endoscopic biliary stenting.
